Awake Craniotomy is a pinnacle of modern neurosurgery. If a patient is asleep under general anesthesia, the surgeon cannot know if a tiny cut is damaging speech pathways. By keeping the patient awake and talking during the critical tumor removal phase, surgeons can safely extract the maximum amount of cancer while guaranteeing the patient's neurological faculties remain perfectly intact. The patient is initially sedated while the scalp is completely numbed with local anesthesia (the brain itself cannot feel pain). The skull is opened. The sedation is turned off, and the patient wakes up completely comfortable. A neuro-psychologist sits with the patient, asking them to count, identify pictures, or move their fingers. Concurrently, the neurosurgeon uses a mild electrical probe to map the brain surface. If probing a spot stops the patient's speech, the surgeon marks it as a "no-go" zone. The surgeon then meticulously removes the tumor around these functional zones. Once the tumor is out, the patient is put back to sleep for the skull closure.
