Percutaneous Nephrolithotomy (PCNL) is the standard of care for treating massive kidney stones (larger than 2 cm) or complex staghorn calculi that fill the entire renal pelvis. Trying to laser such large stones from below (via RIRS) would take too long and risk severe infection. Instead, PCNL offers a direct, fast route. The surgeon creates a tiny tract directly through the skin of the back into the kidney, allowing the insertion of a larger nephroscope and powerful pneumatic or ultrasonic lithotripter probes. These heavy-duty tools rapidly jackhammer the massive stone into pieces and vacuum them out simultaneously, achieving complete stone clearance in a single setting. Advanced Indian centers now offer "Mini-PCNL," utilizing even smaller tracts for faster recovery.

Conducted under general anesthesia with the patient lying face down. Using live X-ray and ultrasound guidance, the urologist precisely punctures the back with a hollow needle directly into the targeted kidney chamber. The tract is dilated to about 1 cm (or 0.5 cm for Mini-PCNL) to allow the surgical scope to enter. The stone is fragmented using pneumatic/ultrasonic energy and the pieces are pulled out with graspers. A temporary drainage tube (nephrostomy tube) may be left in the back for 24-48 hours.
