Radical Cystectomy is the primary curative treatment for muscle-invasive bladder cancer. Historically, removing the bladder meant the patient had to wear a permanent external urine collection bag on their stomach (ileal conduit/stoma). Today, top-tier Indian urological cancer centers offer advanced Orthotopic Neobladder Reconstruction. After the cancerous bladder is completely removed (often using minimally invasive robotic surgery), the surgeon cuts a 20-inch section of the patient's small intestine, slices it open, and meticulously sews it into a spherical pouch. This "new bladder" is attached directly to the urethra, allowing the patient to urinate naturally and maintain a completely normal external body image.

This is a complex, 6 to 8-hour operation performed under general anesthesia. It can be done open or robotically. Step 1 (Excision): The surgeon removes the bladder, surrounding lymph nodes, and nearby reproductive organs (prostate in men; uterus/ovaries in women) to ensure complete cancer clearance. Step 2 (Reconstruction): A specific segment of the small bowel is isolated while keeping its blood supply intact. The intestine is folded and stitched into a round reservoir (neobladder). The ureters (tubes from the kidneys) are plugged into the top of this new pouch, and the bottom is sewn to the urethra.
