Performed under general or spinal anesthesia. A surgical team works simultaneously: one surgeon harvests a small strip of mucosal tissue from the inner cheek (which heals naturally in a week), while the lead reconstructive urologist makes an incision in the perineum (between the scrotum and anus). The scarred, narrow urethra is slit open lengthwise. The cheek graft is meticulously stitched over the defect to instantly widen the tube. A catheter is left in place for 2 to 3 weeks while the graft establishes a new blood supply and heals watertight.
